The Rizzardo Health & Wellness Centre offers a variety of healthcare services close to home. The building is located at 7325 Yonge St. in the Town of Innisfil. Building hours vary to meet healthcare providers’ business hours.

Georgian Nurse Practitioner-Led Clinic

The Georgian Nurse Practitioner-Led Clinic is now open at the Rizzardo Health & Wellness Centre. The clinic will gradually accept patients over the coming months, prioritizing local residents without a primary care provider and equity-deserving populations.

Residents interested in receiving care from the clinic must register through Health Care Connect (HCC). Registration cannot be done directly through the clinic.

Horodynsky Community Kitchen

Located near the front entrance of the Rizzardo Health and Wellness Centre is the Horodynsky Community Kitchen. We hold regular community cooking programs to support learning about nutritious and affordable meal options while bringing community members together!

Community support

The Rizzardo Health & Wellness Centre would not exist without the support of our community. We are grateful for the support of lead donors Sandra and Diego Rizzardo and Boris Horodynsky. The Rizzardo family’s passion for our community was instrumental in helping us break ground on the building, and Boris’ generosity has turned our community's vision of creating a community kitchen into a reality.

From pocket change, to special events, to major donations, the community has generously supported this project, recognizing the importance of having healthcare services closer to home.

Background
  • 2011 - The Innisfil Community Health Committee (ICHC) was established by Innisfil Town Council to gather data to support a business case for public and/or private funding that would address the healthcare needs in Innisfil.
  • 2012 - The ICHC partnered with York University to complete the Innisfil Healthy Communities Assessment Report. This report provided a solid base of research that the ICHC used to direct their activities in the following years.
  • 2014 - The ICHC partnered with the Georgian College Research Analysis Program to prepare and analyze a survey. The data collected revealed the health status of Innisfil residents, residents’ patterns of accessing primary care, the barriers to health services, and the health service needs.

The data showed that nearly half of the respondents have one or more chronic diseases. Furthermore, less than 9% had a doctor located in Innisfil, with 40% of respondents travelling more than 30 minutes to obtain primary health services. The need for improved community health care was clearly demonstrated by these research projects and in response, the ICHC worked with the Town of Innisfil to assess the viability of establishing a community Health Hub.

  • March 15, 2017 = Innisfil Town Council approved the Innisfil Health Hub project to provide health and wellness services in our "backyard".
  • March 1, 2018 - A ground-breaking ceremony was held.
  • May 13, 2019 - The building celebrated a soft opening.
